Fat grafting is a cosmetic procedure where your own fat is used to rejuvenate the face, augment the cheeks or lips, or correct hollowness around the eyes or deep laugh lines. Many people lose facial fat as they age and develop a hollow or gaunt look. Fat grafting can be used to correct this and give a younger, more refreshed look. Fat grafting is also referred to as free fat transfer.

Facial Fat Grafting Surgical Technique
Dr. Sadati says his patients have expressed a desire to avoid using matter, such as implants and repeated and costly fillers, that would be foreign to their bodies when having a facial augmentation procedure. Dr. Sadati’s innovative fat grafting technology allows him to create natural-looking results in less time and with longer lasting results, and is performed under local anesthesia and twilight sedation without the need for general anesthesia.
Facial Fat Grafting During the Surgery
Facial fat grafting with liposuction is performed on an outpatient basis using local anesthesia and twilight sedation without the need for general anesthesia. After the patient is asleep, the sites that fat is being removed by liposuction is numbed with lidocaine. The site that is chosen for liposuction is determined prior to the procedure and marked. Dr. Sadati normally asks patients what area of their body has the most resistance fat to weight changes. The fat that is stubborn and is called a bad fat for patients is a great fat for fat grafting. The amount of time needed for liposuction and facial fat grafting will vary according to several factors, including the amount of fat being removed, the technique used, and the size of the area being treated.The harvested fat is cleansed and purified by centrifuging it.
To ensure survival for grafted fat, about 60 ccs of blood is drawn, and centrifuged using a hospital grade device called Harvest Technology System to isolate the platelet-rich plasma (PRP) layer from the blood cell and pure plasma portions. When ready, the PRP is added directly to the pure fat which contains adult stem cells. This technique provides a very concentrated group of natural growth factors to accelerate the complex healing process which will aid the fat cells to become a viable and permanent tissue.
Then the purified fat is mixed with PRP and carefully injected in very small amount in different layers of facial tissue in order to prevent lumpiness and irregularities and ensure fat survival. Very small blunt needles are used that leave no imperceptible incisions. The procedure is very tedious and precise, similar to sculpting. The procedure can be combined with other surgeries such as a facelift, neck lift, or blepharoplasty. Common areas for liposuction of fat include the abdomen, hips, and flanks.
After surgery, the patient is discharged home. Any discomfort is managed with Tylenol.

Fat Grafting FAQs
Is fat grafting permenant?
It depends. There are many factors involved in how long fat grafting lasts and it appears to be surgeon-dependent. Some surgeons are able to get 50% of the grafted fat to last, but others get up to 90%! There are still some surgeons and patients that express their disappointment with fat grafting because they feel it can be unpredictable or temporary.
Remember that having a successful fat grafting result depends on how delicately it is removed from one area, how well it is inserted into the desired area, as well as having proper nutrients to survive.
I will make an analogy: if you unearth a plant from one side of your yard and then plant it in a new area, without the proper technique to include the roots of the plant, choosing the right soil and fertilizer, the chances for this plant to survive is 50% at best. However, if you were gentle with the removal of the plant, and gave the same plant proper irrigation and fertilization, it would have an 80 to 90% better chance of survival.
Similarly, in fat grafting, if you just take fat from one spot and graft it to another site, the survival rate of that fat is at about 50%.
What is Prp?
Platelet Rich Plasma (PRP) is rich in a variety of valuable natural growth factors such as PDGF (Platelet Derived Growth Factor), and TGF-β1 (Transforming Growth Factor β1). These Growth Factors are known to be important in the initiation and progression of wound healing. These natural growth factors within PRP are also believed to promote and help adult stem cells within fat cells to better differentiate. For this procedure, a small amount of blood is drawn, and centrifuged using a Harvest Technology System to isolate the platelet rich plasma (PRP) layer from the blood cell and pure plasma portions. Facial fat grafting is often combined with other facial procedures such as a facelift, neck lift, blepharoplasty, or brow lift. There is a limit to how many procedures can be done safely, as it relates to surgical recovery and the amount of time the surgery takes.
Return to Work?
Each patient’s recovery time is different and depends on how many procedures are performed at the same time. The average return to work for facial fat grafting alone is less than a week.
