Facelifting isn’t just about pulling skin taut and calling it a day. It’s a craft, a study in human anatomy, and a glimpse into the psychology of beauty. As someone who’s spent decades refining techniques in facial rejuvenation, I’ve learned that a facelift isn’t simply surgery—it’s a collaboration between surgeon and patient, between aesthetics and anatomy. It’s about subtlety, preservation, and, above all, respecting the essence of who someone is.
Let’s rewind to where it all started: the era of skin-tightening facelifts. Back then, surgeons had one primary goal—erasing wrinkles. The method was crude but effective (if you didn’t mind looking like you’d stepped into a wind tunnel): pull the skin back, snip the excess, and stitch it into place. These facelifts often ignored the deeper layers of the face—the muscles, the ligaments, the structures that make us look human.
The result was faces that looked strained and results that didn’t last. Sagging reappeared quickly because the tension was all on the surface. It was like patching a leaky roof without fixing the foundation.
The field changed when surgeons began to understand the importance of the SMAS (Superficial Musculoaponeurotic System). This fibrous layer beneath the skin plays a crucial role in facial aging. Instead of simply stretching the skin, surgeons started repositioning and tightening the SMAS, achieving more durable and natural-looking results.
But even then, it wasn’t perfect. Too much tension, even on the SMAS, could lead to complications or a look that was still slightly “off.” It’s like seasoning a dish—you can’t overpower it. Balance is everything.
That’s when I got to work. I saw the potential to improve these methods and created what I call the Double-C SMAS Plication Technique. Two layers of support that allow the skin to rest naturally without excessive pulling. It’s a method I honed over 1,500 cases, publishing my findings in the American Journal of Cosmetic Surgery.
Not satisfied, I pushed further. The Triple-C SMAS Plication Technique added a third layer of support, enhancing the longevity and durability of results. Published in the Aesthetic Surgery Journal, this technique became a cornerstone of my approach, ensuring outcomes that age naturally with the patient.
Facelifting hit its stride with the Preservation Deep Plane Facelift, a method I’ve embraced and refined in my practice. This technique doesn’t just lift; it restores. By working beneath the SMAS and preserving the skin’s blood supply, this approach minimizes trauma, speeds recovery, and creates a look that’s undeniably natural.
It’s not about sculpting a new face—it’s about uncovering the face you thought you’d lost. The Preservation Deep Plane Facelift addresses deep nasolabial folds, midface sagging, and neck laxity all at once, without the need for surgical drains or excessive undermining. This results in a refreshed, vibrant appearance that feels like you.
Here’s the thing: modern facelifting isn’t just about aesthetics. It’s about the experience. It’s about making procedures safer, reducing recovery times, and giving patients confidence without compromise.
I perform these techniques under twilight sedation—no general anesthesia required. This isn’t just a nod to safety; it’s about making the process more human. Patients wake up feeling like themselves, not groggy strangers in their own skin.
As I look to the future, I’m driven by the same philosophy that brought me here: blend the science of facial anatomy with the art of subtle transformation. Whether it’s through academic research, sharing my techniques at global conferences, or sitting with a patient in my office, my goal remains the same—to help people look and feel like the best version of themselves.
If you’re ready to embrace this next generation of facelifting, let’s have a conversation. Because the art of preservation isn’t just about what’s in the mirror; it’s about the story your face tells and how you feel living it.
